CONSENT TO PROCESSING IMAGE

I consent to the processing my image by Jan Kochanowski University of Kielce, recorded during the Conference "Current opinion in diabetes - challenges and hopes for the 21^st century", hereinafter referred as "Conference", which is held on 1^st June 2022 from 15.00-18.00 (online).

This consent:

1. includes the use of my image to conduct an online broadcast and present a report of the Conference, in particular the publication of my image on the Internet (e.g. www.ujk.edu.pl, cm.ujk.edu.pl, https://currentdiabetes.ujk.edu.pl/), streaming services (e.g. www.youtube.com, Microsoft Teams, Zoom), on social networks (e.g. www.facebook.com),
2. there is no remuneration for granting consent,
3. is indefinite,
4. is territorially unlimited.

INFORMATION CLAUSE

Pursuant to Art. 13 (1) and (2) of the Regulation of the Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of persons related to data protection on data protection 95/46 / EC (Journal of Laws EU L 119 of 04/05/2016 , p. 1, z.) - GDPR, we hereby inform you that:

1. the administrator of your personal data is the Jan Kochanowski University in Kielce - UJK, Żeromskiego Street 5, 25-369 Kielce, e-mail: kancelaria@ujk.edu.pl.
2. the legal basis for the processing of your personal data is:
   • - art. 6 (1)(a) GDPR (the data subject has given consent to the processing of his or her personal data for one or more specific purposes),
   • - art. 6 (1)(c) GDPR (processing is necessary for compliance with a legal obligation to which the controller is subject),
3. Your personal data will be processed in order to organize and conduct the Conference, as well as to conduct an online broadcast, to present a report from the Conference and for billing, archival and statistical purposes.
4. providing your personal data is freely given, but necessary to participate in the Conference. In relation to the image, providing data is freely given.
5. your personal data will be processed for the period necessary to implement the above-mentioned purposes or until the consent is withdrawn, in relation to data processed on the basis of consent and the fulfillment of the archiving obligation resulting from legal provisions.
6. recipients of data may be entities authorized under the law and entities processing personal data at the request of the administrator in connection with the performance of the tasks entrusted to them on the basis of the concluded data entrustment agreement.
7. You have the right to:
   1. access to your data and receive a copy of it, rectify it, delete it (in a situation where data processing does not take place in order to fulfill an obligation resulting from legal provisions) or limit the processing of personal data, and
   2. the right to transfer data and the right to object to the processing - in the cases and on the terms specified in the provisions of the GDPR.
8. you have the right to withdraw your consent; withdrawal of consent does not affect the lawfulness of data processing based on this ground, which took place before its withdrawal; the consent may be withdrawn by writing to the following e-mail address: kancelaria@ujk.edu.pl or to the above-mentioned Postal address,
9. you have the right to lodge a complaint with the competent supervisory authority - the President of the Personal Data Protection Office, when it is justified that your personal data is processed by the controller contrary to the provisions of the General Data Protection Regulation.
